In consistency with Kotlin/KEEP#45

I still believe that we'd rather have an annotation for suppressing this but we gotta start with something.

Also it makes sense to support internal attribute.

so far everywhere VisibilityModifier.DEFAULT is passed.
Those are preparations for supporting #124

Next step would be to customize this via command-line flag

How is it going now? I'm very happy if dukat can support the explicit api mode.
