Charge patron a fee at receive

This implements step b) of specification.

A fee for inter library loan/copy that is submitted at registration of arrival should charge the patron’s account. At registration of arrival, the staff should be able to choose between “fee” or “no fee” and to insert the eventual amount. Default value should be ”no fee”.

I have a hunch libraries might want to be able to do one click and then have the "Type" filled in with "ILL" and a standard amount set (but which can be changed manually). But that is not in the description, so this should cover the spec.
