Insurance types - help with interpretation

There are several types of insurance listed. I understand medicare, medicaid, and that there may be other government insurance (perhaps state insurance programs?). What is self-pay versus private? We wanted to use medicaid as proxy for socioeconomic status, but unclear what the other types might mean (coming from Canada where this is not a thing). Thanks!
