rewrite the worst of the outdated instructions for porting a new model

update the instructions for using the template directory to port a
new model, including reading NetCDF files directly, updating the names
of the relavant namelist items, and making the names of the required
subroutines match the current requirements.

also removed some confusing sub-bullets from the models list.
