cost table: guidance to represent capitation monthly cost (for primary care) #189

Issue Name: capitation cost

Issue Type: Lack of an ETL convention for a CDM table and/or field

Description: See current CDM spec

Suggested solution: Part of cost of healthcare for a patient are monthly pre-payment for primary care cost made to primary care doctor (e.g., in Czechia). I propose there to have guidance under which cost.cost_type_concept_id (in ETL Conventions) in CDM spec to have such thing represented. The health plan pays to a provider a monthly payment and if comparing cost of care between countries (an important research use case for CDM to accommodate) this is useful to have convention for. I propose a direct comment in the specs. As alternative, a convention (per decision of the CDM workgroup)

This concept is close but the price is in fact decided by payer/insurance company.

The proposal is to create a new concept with concept_name of 'Capitation' or 'Capitation cost' or 'Capitation payment'
