Add ybelenko as php-slim4 template creator (#9754)

Thanks to Julian(jfastnacht) who started Slim3 generator and maintained it until Nov 27, 2017(latest PR). Since Slim4 codegen is based on deprecated Slim3 I greatly appreciate your commitment.
