Release 0.95 IDCR version

Have updated this release version to be oriented towards IDCR Integrated Digital Care Record (Professional) oriented functionality, not PHR (Patient /Person Held Record functionality - which was in earlier release for Helm version)

Note the rename of main folder, from Helm
Note the Role Based Access Control changes, with default user being IDCR not PHR user
Further authentication changes to follow as we add further functionality for Showcase stack functionality for Ripple Foundation
