Step to reproduce:

  1. Establish connection to Source Control
  2. Go back in Source Control Settings screen, click again "Accept Settings", the plugin is re-inited which duplicate source control menu entries.
  3. Disconnect from Source Control
  4. Then try to open the Source Control menu
