The discussion in TedDriggs#13 ended up with some API changes. Update the documentation to reflect those changes.
This means that we don't have to print out the inner error in the `Display` impl, as error reporting tools can follow the chain of errors.

Thanks for releasing version 0.4! Noticed there were a few places where the docs needed to be updated; fix those. (Also include an
Error::sourceimpl.)
