Implement a safety database documenting miss/near miss incidents + corrective actions taken in the team, e.g. the 2015 zinc air trash can fire.

@victorchiew + @jd4900 to take charge, discuss implementation details with brandon and @jaygdimayacyac.

Let me know your thoughts on this, we could make a quick one in ruby or something else like flask, or nodejs

* first attempt

* fix silly mistake and edit formatting

* rename to loss_incidents, format code for readability

* add information concerning reporting

Webview is good but currently the mobile experience for submitting an incident is poor.
