* first attempt * basic reporting and database system for safety incidents (hidden from website for testing/ easy suggestions) (refs #254) * fix silly mistake and edit formatting * rename to loss_incidents, format code for readability * add information concerning reporting * html formatting * last attempt before bed * change tags * Formatting webpage with html

Implement a safety database documenting miss/near miss incidents + corrective actions taken in the team, e.g. the 2015 zinc air trash can fire.
