This is a problem that's existed for a long time, and long story short: when a game is saved, its DSP effect isn't saved along with it, so if a player has a DSP effect applied through an env_sound entity, when any save is loaded (or the player quick saves and quick loads), the game just reverts back to having no DSP effect applied at all. Currently the only way to fix this is to go back to that env_sound entity that applied the effect after loading a save.
