Documentation and extended example of AMI usage

As it stands, the AMI is currently only used as a "plumbing" component in support of this example app.

AMI usage and contents are not documented anywhere.

It would be nice if there was a worked example of how to use the Wallaroo AMI to run an example app in AWS -- additionally, it could serve the role of a sanity check during release testing.
