This was mostly about fixing up links and definitions, and adding ids and levels to headers. I extracted the cross-spec links into anchors.bsdata when they weren't already in Shepherd. The second half is in a separate commit so GitHub will show it.

Yeah, @tabatkins couldn't find a Python highlighter that worked on code containing markup. I've filed speced/bikeshed#451 to ask him to try again. |

Bikeshed does a better job of linking to other specs, has an active maintainer, supports things like at-risk paragraphs, and doesn't have a FOUC while it loads.
