Delete the "Example" vhost

As the title says: the "Example" vhost should be deleted (and never created). Further, each vhost home dir should get its own ACME well-known directory instead of centralizing it in the "Example" vhost. We're doing DNS-based ACME, not HTTP-based. Delete it all.

If it is a security concern to allow vhosts to encapsulate their own ACME dirs, then there should be a formal location for them instead of putting them under the confusingly named "Example" virtual host that sounds like it should be deleted.
