As a DeskTop window is being redrawn the cursor appears to be hidden/shown for each icon. If correct, this would impact performance. The cursor should be hidden before each window redraw, shown again afterwards.

Cursor draw is pretty optimized, so this is not a big perf hit.

Introduce inxy macro, which increments as a 16-bit value

This pattern is one byte shorter than CLC / ADC #1, and is used
in a handful of places already. Introduce a macro so it's clearer
what is going on.

No functional changes.
