Combo boxes in settings are not accessible with the Jaws screen reader #76

At the moment, it is not possible to access the text in combo boxes with a screen reader.

Expected behaviour: when selecting a combo box in the settings, the screen reader should read out its value.
Actual behaviour: The screen reader reads out what I think is the element ID when a combo box is selected

Also, combo boxes are now populated after their initialization rather then when opened for the first time (this addresses issue #76)

This should have been fixed in the new version 1.10.11.
