Configure first and last name on the label #533

For some (mostly business-related) events, it would be better to print:

Last Name
First Name
Company

instead of

First Name
Last Name
Company

We should allow the organizer to customize also the first and second row of the label

fixed. Added also the possibility to display a checkbox, which can be useful for marking "goodies collected"
