This is a resubmission of #565 against master.
Note that this doesn't apply the "friendly" names to for composite field types and those are still produced with numeric indexes (e.g., PID.PatientIDInternalID.CX.1). While that's super useful, the only way to get at that with the underlying HAPI library is via reflection, so it will require a separate commit with more extensive testing.
