[chain] Allow Transactions to output Multiple Outputs

This will allow Programs to emit arbitrary events that can be indexed by external providers. It should be possible to emit multiple Outputs because Actions may invoke multiple Programs and/or there may be multiple Actions in a Transaction (#335).

This issue has become stale because it has been open 60 days with no activity. Adding the lifecycle/frozen label will exempt this issue from future lifecycle events.
