Add troubleshooting steps for lights #427

I previously tried out Adaptive Lighting over a year ago, and walked back because I had issues with lights not responding or turning themselves on. Since then, I've figured out the underlying causes, and have been happily using Adaptive Lights for the past month.

My hope is this will help reduce support requests for things the integration can't fix. These could potentially be referenced in an issue template as well.

Unrelated, but I think the README could benefit from a table of contents. I usually use https://github.com/thlorenz/doctoc. If there's interest, I'm glad to do that in this PR or in a new one.
