feat: change form content to match new requirements

Description

• Remove the following exemptions from NIR:
  • Duplicate SRCR - previous submission
    • https://github.com/bcgov/nr-epd-digital-services/blob/dev/forms-flow-ai/epd-forms/bundling/Notification%20of%20Independent%20Remediation.json#L8555
  • Determination that the site is not contaminated
    • https://github.com/bcgov/nr-epd-digital-services/blob/dev/forms-flow-ai/epd-forms/bundling/Notification%20of%20Independent%20Remediation.json#L8565
• Update Schedule 2 activities to match this document.
  • https://github.com/bcgov/nr-epd-digital-services/blob/dev/forms-flow-ai/epd-forms/Site%20Disclosure%20Statements.json#L8345
  • Omit the options marked as "Repealed" (E10 and E12)
