Balance is not pretty much externalized.

Having split up into components, and extracted the b-tree balancer, it
is now possible for someone to write their own balancer, more or less,
and not really, but what's blocking now is the general disarray of the
other components, not the fact that the balancer is tightly bound to
other tree operations.
