Token instance fetcher batch size and concurrency env vars

Motivation

Batch size and concurrency for Token instance fetcher are hardcoded.
Docs update in blockscout/docs#123.

Changelog

Introducing INDEXER_TOKEN_INSTANCE_BATCH_SIZE and INDEXER_TOKEN_INSTANCE_CONCURRENCY env vars.
