Improve test coverage

Current "100" strategy is misleading, since those numbers were achieve by copious use of ignore exceptions, and never included all package sources.

• make coverage levels honest; this will also make over time analysis, like speculated about in Add coveralls integration #34, more meaningful
• shift to all sources mode, more possible since we've dropped demo code in Drop archery demo build solution #43
• as a temporary measure, disable per-file checking, since only global number are in an enforceable range currently
• drive coverage back up by testing through the command module ala Add a kni command integration test #48
• a kni-test.js mode to cover describe.js Add kni describe mode tests #52
• a kni-test.js mode to cover compiled js mode Cover kni command compiled json mode #53, addressing a latent bug found while linting
• another likely milestone will be to then exclude files like html.js until we can get a headless browser test going
• similarly we'll need to devise an interactive mode for kni-test.js to get coverage of readline.js

Without --all we're in the low 90%s ( see #50 )

After #48 we'll at least have coverage of kni.js and maybe others like readline.js

Past that we can add file exclusions for the browser bits, until we can add something like a fake dom or headless browser test.
