Looks like you got into some sort of weird condition when dragging a tab. The ghost image should only appear when dragging a tab out into space and should go away when you release it (creating a new window) or move it back into the tabbed pane.
No known cause, so effectively random. It happened three times across two different MacOS machines. I agree it looks like the client thinks the tab is being dragged to a new location, but no tab dragging was involved- no button being held, just moving the mouse. Clicking off the client or along the tab bar to try and place the tab in a new (or the same) location had no effect, since the tab would only appear when the mouse cursor was on certain areas of the desktop. If I remember right, anywhere off the client showed the ghost tab. A few small areas of the client would show the ghost tab, but the client otherwise worked normally.
We can probably classify this as an intermittent windowing bug.
Mystery bug that causes a transparent workflow window to attach itself to the cursor in some areas of the screen.
