Add question about membership to Self Organized workshop form #2074

On the self organized workshop form, we would like to see the following question:

Is this workshop being organised through a Carpentries membership? (yes/no radio buttons)

This question should follow the question about institutional affiliation and department and be included inside those horizontal lines.

Closing this is it will be addressed in the member codes project.
