Change to 2nd edition of textbook

This PR does the following:

• update link to 2nd edition of the book
• update page numbers, chapter numbers, and chapter titles
• change "module" to "episode" for consistency with Carpentries terminology
• remove repetitive "Discussion Question" from episode titles
