What's the best way to upgrade dokku using dokkufy?

I see there's a server:upgrade command but it's not documented and wasn't working for me, giving me these errors:

ssh: Could not resolve hostname server: nodename nor servname provided, or not known
lost connection
ssh: Could not resolve hostname server: nodename nor servname provided, or not known
lost connection
ssh: Could not resolve hostname server: nodename nor servname provided, or not known
lost connection
ssh: Could not resolve hostname server:upgrade: nodename nor servname provided, or not known

Alternatively, what's the best way to upgrade dokku that's been installed by dokkufy? I'm not even sure where to find the files!

I still couldn't make server:upgrade work but I've been upgrade it manually using the instructions at: http://progrium.viewdocs.io/dokku/upgrading
