Would it be hard to add rtf support? Afaik, iText supports it.
If possible, where would on start?

I switched to use OpenPDF which is a maintained fork of the last open source version of iText. I haven't looked what the state of rtf support is like there. If it's possible you'd probably want to start around here in clj-pdf.
