dcm2mnc open PREVENT-AD 2019-04-13 bug fix release

This tag represents the code changes used to produce the dcm2mnc binary that fixed an issue encountered with DICOM headers having two ASCONV headers that were not properly converted.

This version of dcm2mnc was used to produce open PREVENT-AD MINC files.
