Improve code formatting

Refactor the code so that:

• Spaces are used instead of tabs, with an indentation of 2 spaces;
• Lines do not exceed 79 characters;
• Include order;
• using namespace std; is removed (see: http://stackoverflow.com/questions/1452721/why-is-using-namespace-std-considered-bad-practice)
• Remove commented out (redundant) code;
• Replace #define with anonymous namespace static const;
• Catch exception by const ref;
• Replace boost::foreach with c++11 ranged for loop;
