Replace the "Report a pothole" with GEO-FORM & map

I'd like to work on replacing the existing form on city site with a geo form and map
This would allow users the ability to point on a map and report a pothole. The spaital locations can be stored and housed for analysis and worked on by city maintenance. This would reduce the amount of error in a hand typed form which can have clerical errors in address.

Sounds like a good project! We won't have access to the city site itself ... would you plan to stand up a micro-site that sends form POSTs to the live city site?
