Disable button for edit file name if same

Describe the bug
If the new file name is the same then disable the update file name button. Also don't show the buttons while loading

To Reproduce

1. Go to https://care.coronasafe.in/facility/e02905de-2e0e-41ff-abd1-2eb169a509bc/patient/2db5d708-b2e0-4beb-9c55-fde58131ee6d/consultation/d6d2a4ef-a3cd-4f0a-9618-fa5a02878719/files
2. Click on Edit FIle Name
3. Don't change the file name
4. Click Proceed

Expected behavior
The Proceed button should be disabled
