Disable Auto-copy of medicines to patient discharge pop-up

Describe the bug

When we are discharging a patient from the care, through the patient consultation page, in the discharge pop-up, the medicine is getting auto-filled in the discharge advice and discharge prescription fields with the data from the consultation form.

To Reproduce
Steps to reproduce the behavior:

1. Go to https://care.coronasafe.in/facility/a7651c6a-a9b7-4ab7-8efa-4ccabf8c9009/patient/7d040e34-91de-42a5-894d-9597b0d26682/consultation/acd30290-76d0-496d-ada7-5f77133fa976
2. Click on "Discharge from care"
3. See error

Expected behavior

the discharge advice and discharge prescription fields should not auto-fill data's from consultation form
