Patient Category - Shifting form

Describe the bug
Unable to select category in shifting form

To Reproduce
Steps to reproduce the behavior:

1. Go to https://care.coronasafe.in/facility/6dca552f-aba1-4ca5-afc1-a71744fc3d29/patient/4cdb4510-bfe5-4f41-b1d9-c0fdd27f77ad/shift/new
2. Click on patient category
3. try to change the category
4. the field appears blank and doesn't take any input

Expected behavior
User must be able to change patient category in the shifting form, which should reflect in shifting details and in patient dashboard

Proposed Solution
Remove shifting.patient_category
Use last_consultation.last_daily_round.patient_category ?? last_consultation.patient_category instead
