Add Consultation is permanently disabled in Patient Details page

To Reproduce
Steps to reproduce the behavior:

1. Open a discharged patient consultation
2. Go to patient details page of that consultation
3. Attempt to click Add Consultation

Expected behavior
Add Consultation button should not be disabled and should be able to create a consultation if there is no active consultation (when last_consultation is null or discharged)
