Describe the bug

The patient who are home-isolated are unable to be discharge from care

To Reproduce
Steps to reproduce the behavior:

  1. Go to https://care.coronasafe.in/facility/05fcf441-48b2-4d90-9389-f9ab9af107d5/patient/cb4abc41-0774-46f4-96d0-70910b838f03/consultation/8c3e2fca-bc7b-4031-a808-3b51574f816d
  2. Click on discharge from care and discharge the patient
  3. See error

Expected behavior

We should be able to discharge a home-isolated patient from care
