Procedures in consultation form not working in edit consultation form

Describe the bug
When we edit a consultation, existing procedures are not shown in the procedures section. And hence when we update the consultation, user may accidentally remove existing procedures when the form submits as it's an array field.

To Reproduce
Steps to reproduce the behavior:

1. Edit a consultation that has procedures
2. Observe no procedures shown in edit form although shown in consultation dashboard

Expected behavior
Existing procedures should be shown in edit consultation too
