Use minutes instead of decimal fractional hours. #116

Fractional timezones are formatted with +h.mm instead of decimal hours. This
follows ISO conventions as well as existing ofxtools and ofxgo practice.

Also allow for tzname() to return None.

Fix a coverage problem by asserting that utcoffset is not None. The callers will throw a proper exception.
