Updated docs and added brief troubleshooting section

Hi @djkormo,

Thanks for maintaining this. It works well and I'm glad to have it available. As I was going through setup, there were a couple bits of information I did not find easily in the docs. I've added those in this PR. There was also an out-of-date organization item in a sample certificate request that I updated.

I note you are moving to github pages for docs. If the source for those is available, I'm happy to submit a PR for those or update this one.
