Convert Microsoft Equation Editor equations into MathML

This release supports the conversion of Microsoft Equation Editor equations into MathML, so that they can be displayed using MathJax. You will need to add '<math' as an additional equation delimiter on the MathJax configuration page (Site administration > Plugins > Filters > MathJax).

All-or-Nothing Multiple Choice questions, large Word file imports

This release supports the import and export of the non-standard All-or-Nothing Multiple Choice question type to and from Word. However, a custom version of this contributed question type is required in order to make use of it. Contact me if you want it.

A number of fixes for the Essay question type are also included as the 'Require Text' file is supported in Moodle 2.7 and 2.8, not just 2.9.

Finally, the Word import process now asks for as much memory as PHP is willing to give it, to maximise the size of Word files that can be imported. For example, a Word file containing 1784 questions (without images) has been successfully imported on a test server, but Your Mileage May Vary (YMMV).
