Toggle table of contents from default (as decided by screen width) to force it on or off

On narrow devices:

Table of contents button that toggles table of contents overlay, you select a message, and it disappears (and jump to the message)
- Maybe should float it?
Appear/disappear animates sliding, so you can discover swiping gesture:
Swiping from right side also brings it up
Swiping back hides it

Table of Contents finally has a toggle! Let me know if you have comments on how to format it better.
