This PR is on top of EIP 23 (PR #21) so it includes both changes.

Summary

Removes no longer used functionality and code

Removes no longer used functionality and code in preparation for the migration to Continuous ESD

Removes functionality related to buying and redeeming coupons; redeeming underlying is unchanged from EIP23
Removes getters and setters that are no longer used
Updates and removes tests
