Reduced size of web distribution from 2.1mb to 1.4mb, and switched over to a github source that serves it gzipped - coming out to just ~160kb in the end.

Add R4 and autoconversion support

This release adds R4 support alongside existing STU3 support, as well as support for autoconversion. Autoconversion (enabled by default) will try to use all FHIR versions, starting from R4, to convert the data until one works without any issues.
