This gets rid of the following type of warnings in the documentation:

relative links in Contributing.md dont work in sphinx, since it is included into a different file
follow a style guide for markdown in contributing.md (empty lines around headings and lists)
fix a table not being properly linked in the subduction initiation cookbook because of a double empty line (sphinx seems a bit picky in this case, double empty lines after figures or tables somehow break them).

@tjhei: This should allow #5374 to move forward.
