500 internal server error due to invalid HTML template in enroll, login , challenge form

Describe the bug :

the fix provided in github issue 9000 (No dot display "Internal Server Error") not working as expected.

The original error cases were resolved but now show the customizable "Error" form

However, if there is a malformed HTML in one of the login forms (Enroll, Login, challenge etc) the "Internal Server Error" page is still presented.

The below is an example of a form html error, confirmed produces the “Internal Server Error”.

Using this on teh MFA Enroll form/template.

So, the div was intentionally left not closed in order to break the form. This is what triggers the “Internal Server Error”

Tested on release 3.20.5

This issue has been fixed in versions 3.21.7, 3.20.12, 4.0.2, 4.1.0
