DCR Provider Does Not Appear in UI

Describe the bug

In v4, DCR provider exists in the database but does not appear in the UI.

To Reproduce

Steps to reproduce the behaviour:

1. In a 3.20.x install, create a DCR provider. It will appear in both the database and show in the UI.
2. Upgrade this install to v4. On the client registration screen, the DCR provider shows as empty, but in the database, the provider still exists (and a new provider cannot be added).
3. In a fresh v4 install, create a DCR provider. Back on the client registration screen, the DCR provider continues to show as empty, but there is now a provider that exists in the database.

Expected behaviour

Existing DCR provider should appear in the UI.

Current behaviour

When there is a DCR provider in the database, it does not show in the UI.
