Decouple receipt of metadata from consumption of iterator in SingleThreadedRendezvous

At least one response must be consumed before initial metadata becomes ready for consumption in the current implementation of _SingleThreadedRendezvous. It should be possible to read the initial metadata without consuming any messages from the iterator.
