User and Organization should have a field client_id that indicates the client that created them.

Lastuser also needs API endpoints for client apps to introduce users and organizations.

This has caused some ambiguities in relationships, so re-opening until we fix the relationships.

Remove unused client_id in user, organization, team
338cf96 
Also remove `user.referrer_id`. These columns were introduced for tickets #44, #59 and #84, but never used.
