fix: recognize references in lists and other complex types

This still doesn't mean we can recognize references in arbitrarily deeply nested expressions - that will be addressed in #496, but this PR at least addresses the lowest-hanging fruits.

Brings in the following upstream PRs:

• Reflect that there can be multiple targets at a position hcl-lang#73
• Reflect that ref origins have multiple constraints hcl-lang#74
